Licensing & Application Requirements
Qualifications
- Licensed aestheticians and cosmetologists in Arizona do not need to register separately to perform eyelash extension services (effective September 2024)
- If you are not a licensed cosmetologist or aesthetician, you must meet one of the following:
- Provide proof of completing a lash training program before October 30, 2023
- OR complete a Board-approved eyelash technician training program
Important Notice:
Courses taken between October 31, 2023 and May 3, 2024 will not be accepted
A Board-approved training program is required for eligibility
Application Requirements
- Age Requirement — Applicants must be 16 years or older to enroll in the program
- Proof of High School Education for ages 16-18 — Transcript showing at least two years of high school or a high school diploma (or equivalent). If you are 19 years or older, it is not required.
- Training Certificate — Provisional registration certificate received after completing a Board-approved training program
- Proof of Legal Status / Eligibility to Work in the U.S. — Applicants must provide valid documentation showing legal presence and/or authorization to work in the United States. Acceptable forms include: U.S. Citizenship documentation, Permanent Residency (Green Card), Valid Work Permit / Employment Authorization, U.S. Passport or Foreign Passport, Arizona State ID or Government-Issued Photo ID.
- 2x2 Photo — Must be taken against a white background, from the shoulders up, with no filters or edits

Calificaciones

Requisitos de Solicitud

- Certificado de Entrenamiento — Certificado provisional de un programa aprobado por la junta
- Prueba de estatus legal / elegibilidad para trabajar en los EE. UU. - Los solicitantes deben proporcionar documentación válida que demuestre presencia legal y/o autorización para trabajar en los Estados Unidos. Los formularios aceptables incluyen: EE. UU. Documentación de ciudadanía, residencia permanente (tarjeta verde), permiso de trabajo válido / autorización de empleo, EE. UU. Pasaporte o pasaporte extranjero, identificación del estado de Arizona o identificación con foto emitida por el gobierno.
